CASE BACKGROUND
34 year old pregnant female with PCOD and enlarged bilateral ovaries and left adnexal mass
LOP (LUTEOMA OF PREGNANCY)
♦ LOPs are usually an incidental finding in third trimester pregnant patients, usually are found at time of C-section or postpartum tubal ligation

♦ Generally resolve after delivery

♦ Often associated with hilus cell hyperplasia and thus can cause virilization/masculinization in the mother and/or female infant

♦ Microscopically, basically see nodular theca-lutein cell hyperplasia admixed with follicle like spaces

♦ Luteinized theca cells have eosinophilic cytoplasm (smaller than hilus cells), are round to spindled, with single prominent nucleoli (can be slightly pleomorphic with mitoses)
